The challenge
Stavanger Municipality needed a new ambulatory service that gives vulnerable children and families coordinated support across schools, child protection, healthcare and social services. Existing structures were fragmented, leaving families to navigate multiple agencies on their own and increasing the risk of gaps in follow‑up, duplicated efforts and poorer outcomes.
What we did
Over several years, Canoe co‑developed the holistic Team på Tvers service model together with municipal and healthcare professionals and tested with families, teachers and other relevant stakeholders.We mapped user journeys across agencies, ran co‑creation workshops, and designed new routines and tools for shared assessment, planning and follow‑up.We also made the service model scalable by developing a clear framework and implementation manual that other municipalities can adopt and adapt. Our team implemented practical tools for user involvement, systematic needs assessment and shared decision‑making, and worked embedded with municipal teams throughout implementation to ensure the new service could be sustained in everyday operations.
